Patient Forms

No need to fill out paper forms! Eterna Aesthetics patients use our PhreesiaPad when they arrive and complete the electronic check-in interview from the waiting room. Our digital solution collects full patient history, signatures for consent forms and insurance information. Patients can even make their co-payment or pay their outstanding bill.

No Show / Late Arrivals
