Patient Forms
No need to fill out paper forms! Our patients use our PhreesiaPad when they arrive and complete the electronic check-in interview from the waiting room. This digital solution collects full patient history, signatures for consent forms and insurance information. Patients can even make their co-payment or pay their outstanding bill.

Payment Options
Accepted forms of payment include Care Credit, Visa, Mastercard, American Express and Cash.
Fees & Policies
MDC+EA accepts Visa, Mastercard, American Express, Care Credit and Cash for payment of all services. Payment is due in full at the time services are rendered.

NO SHOW / LATE ARRIVALS
We are considerate of your time and kindly request that you arrive 15 minutes before your scheduled appointment to complete the check in process. If you are unable to keep a scheduled appointment, we ask that you provide a 24-hour notice. Patients may be charged a $25 fee to reschedule if less than 24-hour notice is given for appointment cancellations. Additionally, arriving more than 15 minutes late to an appointment may result in a no-show and a $25 fee will be requested to reschedule.
